 Fill out the Medical History Form and e-mail it to Upon completion of your medical history form, you will receive an e-mail regarding further information on programming, laboratory profiles, services, and financial information.

 The patient along w/ clinic consultation will determine the level of lab testing required. Lab orders will be faxed to the lab closest to your home. Billing We can bill our account wholesale contracted rates with LabCorp nationally or Quest Diagnostics in the Midwest. The patient has the option of billing their insurance.

 Following completion of all labwork, results are finalized and you will be scheduled for an appointment with Dr. Fishman. Appointment times range from 1.5 - 2 hrs. (Today, many physicians may see 30-40 patients daily in US healthcare clinics.)
